Instructions

  1. Add your name and email address.
  2. Complete the rest of the form adding your scores indicating how you performed in the training drills.
  3. Add a rating for the coaches.
  4. Add comments about the coaches.

Add an entry to your Hockey Journal for Mon 13th October.


Player Feedback

Choose a score that reflects how well you felt you performed in each Training Drill. Write a comment about each drill (optional).


Warm-up Feedback

Score Range: 1=Found it difficult, 2=I did ok, 3=I felt comfortable, 4=It was easy, 5=I am an expert.


Training Drill Feedback

Score Range: 1=Very Poor 2=Poor, 3=Good, 4=Very Good, 5=Excellent.


Rate the Coaches

Score Range: 1=Very Poor 2=Poor, 3=Good, 4=Very Good, 5=Excellent.
